Somerset ICB Selects IEG4 to transform continuing healthcare services

NHS Somerset has chosen IEG4 to implement a cutting-edge digital Continuing Healthcare (CHC) platform. This move aims to revolutionize the All Age Continuing Care (AACC) process and enhance service delivery across the region

Addressing past challenges

Previously, NHS Somerset encountered difficulties with its existing platform, which necessitated significant manual input, particularly for data entry into property fields.

Benefits of the new platform

The new platform offers a user-friendly interface for both staff and patients. It enables the digitization and automation of referral forms, facilitating real-time updates on progress. Additionally, it provides a portal where users can access administrative and payment tools, offering complete visibility into the complex CHC process.

Streamlined workflows and improved efficiency

IEG4’s CHC platform consolidates multiple processes into a single digital system. This streamlined approach will enhance workflows for staff and facilitate the management of the entire assessment and referral process, including case management for CHC-eligible patients.

Meeting regulatory requirements

The platform aligns with the Patient Level Data Set (PLDS) requirements, enabling NHS Somerset to generate the necessary data extracts for monthly reporting to NHSE.

Enhancing patient experience and efficiency

Lynette Emsley, Associate Director of Continuing Healthcare and Care Sector Services at NHS Somerset, expressed enthusiasm for the new platform: “This will make it easier for individuals and their families to be involved in the CHC assessment process, and for patients and staff to readily access the information they need.”
